Send the completed form with the required<strong> specification <\/strong>to your Laboratory Assistant*. He\/She will advise and assist you with the quotation.<\/li>\n<li>Send the forms and quotation to our financial admin assistant** for the further process.<\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<h2>(b) Chemical supply<\/h2>\n<ol>\n<li>Please fill in this <a href=\"https:\/\/www.ukm.my\/selfuel\/wp-content\/uploads\/2021\/02\/BORANG-A-Pembelian-Bahan-Kimia-ISF-4.pdf\" target=\"_blank\" rel=\"noopener\">form 2<\/a>. <a href=\"https:\/\/www.sigmaaldrich.com\/safety-center.html\" target=\"_blank\" rel=\"noopener\">MSDS<\/a> information would be required to fill in the form or refer to <a href=\"https:\/\/www.ukm.my\/selfuel\/wp-content\/uploads\/2021\/06\/Jadual-1-Senarai-Bahan-Kimia-Berbahaya-Kepada-Kesihatan.pdf\" target=\"_blank\" rel=\"noopener\">Schedule 1<\/a>. Use a single form for each chemical. Kindly send the completed form together with the MSDS to En. Azman (azmannor@ukm.my) for approval.<\/li>\n<li>Once you get the permission to acquire the chemicals, fill in this <a href=\"https:\/\/www.ukm.my\/selfuel\/wp-content\/uploads\/2025\/04\/UKM-SPKPPP-PP03-BO01-Borang-Permohonan-Perolehan-Bekalan-Perkhidmatan-S01.docx\" target=\"_blank\" rel=\"noopener\">form 1<\/a>. Send the completed form together with the approved form 2 to your Laboratory Assistant*.
