CHECKLIST UKMAEC. Please Click Senarai-Semak-UKMAEC <\/a><\/strong><\/span><\/p>\n

Please ensure that all the documents (FIVE\u00a0 items) below are attached with your application form before submitting it to the Secretariat of Universiti Kebangsaan Malaysia Animal Ethics committee (UKMAEC), application submission is via EMAIL : ukmaec@ukm.edu.my<\/strong><\/p>\n

  1. UKMAEC application form.<\/li>\n
  2. Full publication if any (Minimum 1 publication \u2013 Maximum 3 publications).<\/li>\n
  3. Provide flow chart on summary of study design.<\/li>\n
  4. Provide your contact number and email address.<\/li>\n
  5. Payment Proof.<\/li>\n<\/ol>\n

    *For the application from PPUKM, there is additional Infos required. This is for the Medical Research Secretariat of the Faculty screening procedure.<\/span><\/strong><\/p>\n

    The additional Infos are :<\/p>\n

    1. Document of Borang Permohonan Menjalankan Penyelidikan\u00a0(UKM-SPPI-BO01)<\/em><\/li>\n
    2. Perjanjian Kerahsiaan (Non-Disclosure Agreement) \u2013\u00a0UKM Student ONLY<\/em><\/li>\n
    3. Document of Research Proposal<\/em><\/li>\n
    4. Publication Policy (completed and signed by all researchers involved)\u00a0<\/em><\/li>\n
    5. Research Agreement (Draft – if relevant)<\/em><\/li>\n
    6. Any form of confirmation from the sponsor (outside grant)<\/em><\/li>\n
    7. Checklist for Proposal Acceptance by the office<\/em><\/li>\n
    8. A letter from an outside organisation (other than UKM) for research permission (if relevant )<\/em><\/li>\n<\/ol>\n
