Checklist for Further Action
FACULTY OF INFORMATION SCIENCE AND TECHNOLOGY (FTSM) If you are ACCEPTING the offer, kindly proceed with the following actions:
- Make a minimum payment of 50% from the total amount for the first semester, latest by 22 August 2018. Kindly refer to the Payment Structure and General Rules on Student Finance. Please present the payment slip during registration.
- Perform your medical check-up at a government or private hospital and upload the Medical Report Form before the registration date.
- Student Visa - Please refer to Immigration Information attachment and email the required documents together with proof of payment to firstname.lastname@example.org latest two (2) months before registration day.
- Complete the Library Card Application Form, and send an email to email@example.com or firstname.lastname@example.org after the registration date.
- Clothing must be neat, tidy and presentable during registration.
**Reminder: Jeans, Blouse and Skirt are not allowed.
Note: All students must attend the briefing by faculty at Bilik Mesyuarat 1, Faculty of Information Science and Technology (FTSM) on 12 September 2018, 10.30am.
