Independent and Certified Family Medicine Specialist with interest in Adolescent Health

  1. Competency in managing stroke in primary care with emphasis on transfer of care management, long-term stroke care, multi-discplinary management approach and provision of family/community support
  2. Ability to recognize the complexity of long-term stroke patient and instituting individualized and hollistic management
  3. Ability to lead the community stroke care team in primary care clinic using local and known resources
  4. Develop critical thinking, leadership qualities and self-directed life-long learning using evidence-based clinical management.
  5. Ability to identify and utilize the various resources in the community managing long-term stroke patient in the community.
  1. Active participation in clinical activities including long-term stroke services, chronic care management and neuro-rehabilitation stroke planning.
  2. Active participation in patients’ stroke support group in understanding, planning and managing their activities in fulfilling the principle of community re-integration for long-term stroke patients.
  3. Active participation in the unit’s educational activities including CME, discussion on current relevant evidence, case-based discussion and audit activities.
  4. Attachment in rehabilitation units in Rehabilitation Hospital Cheras to learn about various rehabilitation options in fulfiling the principle of further rehabilitation for long-term stroke patients.
  5. Attachment in Department of Rehabilitation, Hospital Canselor Tuanku Muhriz to understand regarding neuro-rehabilitation services for stroke patients.
  6. Visits to the National Stroke Association (NASAM) centres to understand regarding community stroke care options in Malaysia.
  7. Record experiences in a log-book in terms of cases managed, teaching & learning activities, attachments, visits, research and publication.
  8. Scheduled meetings with supervisor throughout the duration of the fellowship for progress monitoring.
  9. Conducting either audit/research with minimum of one (1) publication and one (1) proceeding at the end of the fellowship.

 

At the end of the fellowship, candidate will receive;
Certificate of completion of training certified by Course Director and Dean, Faculty of Medicine UKM will be given to all successful candidates.

  1. Please apply at least 6 months before intended start date.
    You will need Temporary Practising Certificate (TPC) by Malaysian Medical Council (MMC).

  3. Application for TPC will be forwarded to MMC by UKM except for Letter of Good Standing.

Flowchart of Credentialing and Privileging in HCTM for International Candidates

DEFINITION

Credentialing: a process to set the standards and to do an evaluation for a person’s qualification in a field or procedure. It is recognition for the professional training done by a medical practitioner.

Privileges: the rights given by the hospital for a medical practitioner to give treatments or to do procedures on hospital patients based on the qualifications of the training, and the medical practitioner’s experience and expertise.
