FELLOWSHIP IN INTERVENTIONAL PAIN PROCEDURE INCLUDING ENDOSCOPIC SPINE PROCEDURE (UKM)

FELLOWSHIP IN ADVANCED INTERVENTIONAL PAIN MEDICINE CLINICAL FELLOWSHIP

You will need Temporary Practising Certificate (TPC) by Malaysian Medical Council (MMC). Please ensure you fulfil all the requirements for TPC.

For Malaysian practitioner:

Candidate need to register under National Specialist Registry (NSR) for 1 year with either one of following certifications:

• Master of Sports Medicine, or its equivalent based on the committee.

• Master in Orthopaedic, or its equivalent based on the committee.

• Master in Anesthesiology, or its equivalent based on the committee.

• Board-certified neurosurgeon by NSR who has completed an accredited neurosurgical residency.

• Board-certified MSK Radiologist by NSR who has completed an accredited MSK residency.

For International candidates:

Candidate need to register under their home National Specialist Registry for 1 year with either one of following certification:

• Master of Sports Medicine, or its equivalent based on the committee.

• Master in Orthopaedic, or its equivalent based on the committee.

• Master in Anesthesiology, or its equivalent based on the committee.

• Board-certified neurosurgeon who has completed an accredited neurosurgical residency.

• Board-certified MSK Radiologist who has completed an accredited MSK radiology residency.

The Fellow will be trained in advanced interventional pain management mostly focus on complex chronic pain. This fellowship will focus on getting accurate diagnosis, interpret imaging and perform advanced interventional skills using point-of-care ultrasound (POCUS) or fluoroscopic. The Fellow will assist the doctor in the ward, clinic, and to assist in interventional procedures at the day care or procedure room sessions.

1. To upgrade advanced interventional skills of point-of-care ultrasound (POCUS) and fluoroscopic guided in complex chronic pain.

2. To further enhanced skills in evaluating and treating complex chronic pain.

3. To enhance experience in performing, writing and publishing research related to complex chronic pain.

First Pre-requisite: Specialist at its specialty for 1 year

For Malaysian practitioner:

Candidate need to register under National Specialist Registry (NSR) for 1 year with either one:

• Master of Sports Medicine, or its equivalent based on the committee

• Master in Orthopaedic, or its equivalent based on the committee

• Master in Anesthesiology, or its equivalent based on the committee

• Board-certified neurosurgeon by NSR who has completed an accredited neurosurgical residency

• Board-certified MSK Radiologist by NSR who has completed an accredited neurosurgical residency

For International candidates:

Candidate need to register under their home National Specialist Registry for 1 year with either one:

• Master of Sports Medicine, or its equivalent based on the committee

• Master in Orthopaedic, or its equivalent based on the committee

• Master in Anesthesiology, or its equivalent based on the committee

• Board-certified neurosurgeon who has completed an accredited neurosurgical residency

• Board-certified MSK Radiologist who has completed an accredited MSK radiology residency

Second Pre-requisite: Basic MSK ultrasound and Fluroscopy

Candidate need to attend one basic MSK ultrasound workshop AND fluroscopic workshop recognized by MSIPP.

This Clinical Fellowship programme would allow the Clinical Fellow to:

• Obtain a broad knowledge base of medical issues surrounding complex chronic pain diseases.

• Enhance skills in evaluating and treating complex chronic pain diseases.

• Become proficient in common interventional procedures of point-of-care ultrasound (POCUS) and fluoroscopic technique in chronic pain diseases.

• Gain experience in performing, writing and publishing research.

Method of Supervision:

Direct observation and feedback. Fellow will be always supervised by an assigned supervisor.

Hands on Experience:

Fellow will have opportunities to observe the following procedures:

• USG AND fluoroscopy Guided shoulder interventional - diagnostic block, thermal neurolysis /neuromodulation, barbotage.

• USG AND fluoroscopy Guided elbow interventional - diagnostic block.

• US Guided wrist & hand interventional - diagnostic block, percutaneous release.

• USG AND fluoroscopy Guided knee interventional - diagnostic block, thermal neurolysis /neuromodulation.

• USG AND fluoroscopy Guided ankle interventional - diagnostic block, thermal neurolysis /neuromodulation.

• USG AND fluoroscopy Guided hip interventional - injection, diagnostic block, thermal neurolysis / neuromodulation.

• USG AND fluoroscopy Guided spinal interventional - injection, hydrodissection, diagnostic block, thermal neurolysis / neuromodulation.

Fellows will need to demonstrate their proficiency level based on the following competencies:

1) Patient Care

• Become highly proficient in the evaluation and treatment of patients with complex chronic pain in the clinic.

• Be able to order appropriate diagnostic tests and imaging studies to assist with diagnosis and accurate assessment of the level/severity of the injury.

• Be able to interpret diagnostic test and imaging studies (X-rays, CT scan, and MRI) to assist with diagnosis and accurate assessment of the level/severity.

2) Medical Knowledge

• Develop a sophisticated understanding of the pathophysiology of complex chronic pain.

3) Practice-Based Learning and Improvement

• The Fellow will be expected to formulate a plan of treatment, which will then be reviewed in detail and either confirmed or altered as necessary to achieve optimal patient outcome.

Formative assessment:

Regular evaluation between fellow and supervisor.

Summative assessment:

End of training feedback.

The general overall grading system evaluates the Clinical Fellow's performance upon completion of the fellowship programme. All Clinical Fellow will be given a general overall grading status at the end of the fellowship programme based on the grading criteria requirements incorporating the six competencies based knowledge, skills and performance that Clinical Fellow must demonstrate throughout the programme.

Successfully Complete: At least 90% attendance for all meetings and training activities AND Active participation in most training activities.

Unsatisfactory Complete: Less than 30% attendance for all meetings and training activities OR Consistently demonstrates poor training skills and patient care OR Demonstrates no respect towards Supervisor or other Health Professionals' advice OR Lack of responsibility throughout the programme and lack of interest to learn.

Did Not Complete: Does not attend any meeting or attend any training activities OR No supporting documents for absence or valid reason for not completing the programme (e.g. medical certificate / hospitalisation leave).

Withdrawn Programme: Absolutely nil participation in the programme.

The attachment programme will be terminated early on the ground of the Clinical Fellow's poor performance, misdemeanour, misconduct, negligence or breach of any terms stipulated or referred to in the Clinical Fellowship Letter of Offer and Institution Terms and Conditions.

The Clinical Fellow may also request to terminate the attachment programme for reasons such as serious illness or other personal obligations. The institution will review all requests for early termination with the Clinical Fellow and the Supervisor/ Head of Department.

DEFINITION

Credentialing: a process to set the standards and to do an evaluation for a person’s qualification in a field or procedure. It is recognition for the professional training done by a medical practitioner.

Privileges: the rights given by the hospital for a medical practitioner to give treatments or to do procedures on hospital patients based on the qualifications of the training, and the medical practitioner’s experience and expertise.
